Interprofessional Healthcare Providers (IHPs)

The Family Health Team is a way of providing health care to you and your family.  It is a team approach that brings together family doctors with other health care providers in order to coordinate the best possible care for patients.  Everyone on the team works together to meet your needs.  Their focus is on keeping you and your family healthy, not just treating you when you are sick.

When you enrol as a patient with a Family Health Team, you are taking a step to ensure that you always have ready access to a team of health care providers.  Our team includes:

Nurse Practitioners (NP) provides same day and next day appointments for acute and episodic health concerns while providing leadership and support to OSFHT programs that help patients manage their chronic illnesses. www.npao.org

Pharmacist provides medication-related education and support to patients on medications and those with complex illnesses. www.ocpinfo.com

Social Workers (MSW) help resolve problems by providing counselling and therapy when needed. 

Psychologists provide psychological testing, assessment, and therapy for mental health problems.  www.cpo.on.ca

Registered Dietitians (RDs) provide advice on how nutrition and diet impact on maintaining health, as well as providing disease prevention and treatment services for diabetes, heart disease and other conditions. www.dietitians.ca

Registered Nurses (RNs) provide preventive health examinations, as appropriate, and provide health education and monitoring for patients with chronic conditions. www.rnao.org

Registered Practical Nurse (RPN) can care for patients whose care needs are well defined and the health condition is well controlled. www.rpnao.org

Registered Respiratory Therapist (RRT) Provides treatment or management of acute and chronic breathing disorders. www.crto.on.ca

Occupational Therapist (OT)  Provides services such as cognitive and/or perceptual evaluations, assessment for varying assistive devices, functional assessment, and falls prevention. Service delivery may be offered to individual clients or in a group setting and includes education and consultation services to patients, family members or team members.
