Quality Improvement Plan
Introduction:
The OSFHT strives to continuously improve all our services to create a well-rounded experience for all of our patients. We collaborate with Health Quality Ontario and the South West LHIN to identify priorities for our quality initiatives. This includes:
- Improving access to care
- Care coordination and integration
- Utilising our resources more efficiently and effectively
- Aligning our programs to the needs of our population
- Putting patient safety at the centre of everything we do
- Ensuring an excellent patient experience at the FHT
